  • GP / specialist referral letter

  • Health Fund details
  • Any X-rays and Scans
  • Any past letters from other doctors relating to your complaint
  • Hearing test results
2. Surgery Related

This depends on which hospital you have elected to have the surgery at. Options include:
  • Privately at Sydney Southwest Private Hospital or Campbelltown Private Hospital
  • Be placed on the Public waiting list for the Public Hospital System

  • A quote for surgery will be emailed within the week
  • Follow the instructions on that email
  • Check with your health fund
  • Contact our Anaesthetist for a quote - details will be given to you
  • Sign and return the acknowledgement of the surgeon's fees
  • The consent form is kept by our office and forwarded to the Private Hospital once you have agreed and a date has been decided
  • Complete the online admission forms for the Private Hospital

  • Dr Chin will give you a Recommendation for Admission (RFA) Form with a signed consent
  • Check your personal and contact details are correct
  • Fill in the rest of the RFA Form
  • The RFA form is to be handed into the admissions office at the Public Hospital.
  • This is important - if the RFA is not handed in, you are not on the waiting list. Waiting time starts from the date it reaches the hospital.
  • The hospital will send you a letter of receipt. This may take a few months. If there is no word, please contact them.
  • Wait to be contacted by the hospital regarding the surgical date
  • Note: if you are a public patient, Dr Chin may not perform the surgery but will supervise the surgical trainee allocated to that hospital

For routine surgery, the current waiting time is 12–18 months from the date you submitted the RFA Form to the Public Hospital.

If you are a private patient, Dr Chin will perform the surgery.

If you are a public patient, Dr Chin may not perform the surgery but will supervise the surgical trainee allocated to that hospital.

Sometimes the Public Hospital moves your care to another surgeon who may perform the surgery in that hospital or another one. They should make it quite clear that your care has been transferred. Once transferred, post-operative care should be with the new surgeon.

  • The hospital will contact you to confirm the surgery and date
  • Some patients will be required to attend the Public Hospital Anaesthetic Pre-Admissions clinic (public patient), or you will be contacted by our anaesthetist directly (private patient)
  • Please continue or cease medications as instructed by doctors. If unsure, contact us at least 2 weeks prior to surgery
  • The night before surgery, the hospital will contact you regarding fasting times and when to arrive

  • Be prepared for an overnight stay in hospital (even if you are having day surgery)
  • Bring any scans or X-rays relating to the surgery
  • If your child is having surgery, please bring some toys to keep them occupied while awaiting surgery
  • If possible, try not to bring siblings until after the surgery

No - we may be able to test your or your child's hearing at the time of consultation, or arrange for it to be done at a later date if necessary.
